Enrollment Fee - $35

Facility Upgrade Fund/Maintenance Fee- $50 collected 90 days after the 1st day of your membership.

Freeze Options - $0 for first 4 weeks and $5 for each week past the initial 4 weeks with a 12-week maximum freeze length. Once the 12 weeks has expired, membership will automatically be reactivated or the cancelation process will ensue.

STANDARD $35.00 ENROLLMENT FEE: Applies to all membership programs, is non-refundable, and will be due upon execution of this Agreement unless otherwise agreed upon.

3 MONTH LOVE YOURSELF COMMITMENT: We believe strongly in the Phoenix A+ Coaching Fitness Formula and we take your satisfaction very seriously. By signing below, you are committing to join the Phoenix A+ Coaching Fitness Family for a minimum of 3 Months.

LOVE IT OR LEAVE IT GUARANTEE: You may cancel this agreement at any time before your trial expires without penalty by emailing [email protected].

MEMBERSHIP FEES AND ELECTRONIC FUNDS TRANSFER (EFT): This is a legal contract. After the trial membership offer, you agree to be automatically billed each week for the amount specified and duration specified above. After the initial 3 Month Commitment period, membership will continue, month to month, unless and until you cancel through the cancelation process included in this document.

CLOSURES: I understand that there will be dates that the facility will close for team developments, holidays, snow, dangerous road conditions etc. We will provide at home workouts for you in such events and no credits will be issued for scheduled and unscheduled closures.

CANCELATION: You may cancel at any time for any reason by requesting a cancellation notice via email to [email protected]. In response to your notice, you will be sent the official electronic cancelation form. Membership and payments will continue until the electronic cancelation form is completed and submitted. Regardless of when you submit the official electronic cancellation form, you will be liable for the greater of (a) the remainder of the first 3 months of membership fees or (b) 1 month of membership fees from the date the completed form is submitted. In other words, if you cancel during month 2, you would owe nothing more than the payment for the initial 3 months. If you cancel in month 3, you would be liable for the month. You authorize us to process payments consistent with the foregoing.

Paid in full agreements are non-refundable and non-transferable due to the substantially discounted pricing.

FREEZE OPTIONS: We may allow you to freeze your membership for a limited time (up to 12 weeks) due to medical issues (pregnancy, injury, sickness) upon receipt of a note signed by a medical doctor submitted before the requested freeze date and clearly state that you cannot exercise safely and the expected duration of your absence. The first month of your freeze will be free ($0), each month after the initial month will have a fee of $20 per month up to the maximum of 3 months.

I understand I cannot be credited a freeze (no refunds) if I notify Phoenix A+ Coaching after payment has been drawn. In order to freeze my account (medical freeze) I must email [email protected] prior to the requested freeze date with a signed doctor's note, otherwise freeze will not be granted. Membership Suspension of any kind does not release me from the agreed upon 3 Month commitment or the 30 Day Cancelation Policy.

DECLINED PAYMENTS: Declined credit cards or non-sufficient funds (NFS) are subject to a $15.00 fee.

BUYER'S RIGHT TO CANCEL: Additionally, under Missouri law, if you wish to cancel this contract within three business days of its making, you may cancel by delivering written notice to us by certified mail, return receipt requested. Missouri law requires that the notice must say that you do not wish to be bound by the contract and must be delivered or mailed before midnight of the third business day after you sign this contract. CLIENT AGREEMENT: By signing below, you acknowledge that have read, understand, and agreed with all terms and conditions herein. This Agreement and the Waivers, Releases, And Assumptions of Risk Agreement constitute the entire agreement of the parties and no other agreement or understanding exists between Client and Athletic Training Principles LLC d/b/a Phoenix A+ Coaching.

You agree your electronic signature ("E-Signature") is the legal equivalent of your manual signature on this Document. You agree that no certification authority or other third-party verification is necessary to validate your E-Signature and that the lack of such certification or third-party verification will not in any way affect the enforceability of your E-Signature. You may request a paper version of an electronic record by writing to us. We reserve the right to charge a reasonable fee for the production and mailing of a paper version of the record. Your current valid email is required for all communications.
